What Plans Are Available?

You’re well protected with NVIDIA’s healthcare benefits. We offer comprehensive coverage for you and your family, and we keep it affordable by paying the majority of the cost.

Your Medical Plan Options

You have up to five medical plans to choose from, depending on where you live. Some plan features are standard. For example, all of your options cover preventive care at no cost to you; all offer generous coverage for prescription medications; and all provide the financial protection you need in the event of a major illness or injury.

Other features may be unique to a particular plan. Picking the right plan depends on your healthcare needs and your budget.

The NVIDIA HSA, NVIDIA HSA Plus, and NVIDIA PPO medical plans use Cigna’s Open Access Plus (OAP) Network of hospitals and physicians for in-network services. Participants in MA use the Open Access Plus—CareLink (OAPC) Network. OAP and OAPC participating providers are found on the Cigna website at www.mycigna.com.

The Kaiser HSA and Kaiser HMO use their own network of doctors and facilities. Find participating providers on the Kaiser website at www.kaiserpermanente.org.

The Blue Cross Blue Shield Alabama plan uses a separate network. Find participating providers at www.bcbs.com.

NVIDIA HSA (OAP Network)

Pay $0 out of your paycheck each month! Plus, enjoy robust coverage and all the benefits of the Health Savings Account.

NVIDIA HSA Plus (OAP Network)

With this health plan, you get greater control over your healthcare spending, low payroll deductions, in- and out-of-network coverage, and tax-free savings from a Health Savings Account (as well as a contribution from NVIDIA).

If you live in Alabama, you have access to an additional health plan, the Blue HSA Gold plan. This also includes a Health Savings Account, with a contribution from NVIDIA.

NVIDIA PPO (OAP Network)

This plan offers higher payroll deductions in exchange for predictable in-network costs. It also includes 90% coverage for in-network hospitalization, but no tax savings from the Health Savings Account (and no NVIDIA contribution to your HSA).

Kaiser HSA (California)

All the benefits of a medical plan with a Health Savings Account, with the familiarity of the Kaiser network.

Kaiser HMO (California)

HMO plans offer no annual deductible and predictable copays when you need care. You must use in-network providers for your care.

Blue Cross Blue Shield Alabama Platinum Plan

This plan offers higher payroll deductions in exchange for predictable out-of-pocket costs. It also includes a lower deductible and in- and out-of-network coverage but no tax savings (or NVIDIA contribution) from the Health Savings Account.

Your Dental & Vision Plan Options

In addition to major medical coverage, NVIDIA offers vision and dental benefits to keep your eyes bright and your smile sparkling.

Dental Plan Details

The NVIDIA Dental Plan gives you two options for coverage—a PPO and a DHMO. Both cover in-network preventive care (cleanings, exams, and X-rays) at 100% with no annual benefit maximum. The PPO plan provides three cleanings per year. Details on what you pay for basic care, major care, and orthodontia, both in- and out-of-network, is in the chart below.

NVIDIA Dental Plan [PDF]

The NVIDIA Dental Plan uses the Delta Dental PPO and Premier networks for in-network dental services. Provider lists can be found on the Delta Dental website.

Vision Plan Details

Vision benefits are available through the Vision Service Plan (VSP). You make a small copay at the time of your visit for certain covered services like yearly exams, eyeglass lenses, and contact lenses. You get the best deals when you use a VSP provider, and all you need to do at the time of your appointment is tell them that you have VSP—no ID card is necessary. Your share of the costs for everything from lenses to LASIK surgery is in the chart below.

Your Spending & Savings Accounts

For more financial protection, enroll in one or more of NVIDIA’s saving and spending accounts to complement your optional coverage.

Flexible Spending Accounts

NVIDIA offers three types of Flexible Spending Accounts (FSAs), which allow you to put aside pre-tax money to pay for eligible expenses for yourself and eligible family members (as defined by the IRS):

  1. Health Care Flexible Spending Account (Health Care FSA). Use this account to pay for eligible medical, dental, vision, and pharmacy expenses. You can contribute $120 to $2,600 in 2017.
  2. Limited Purpose Flexible Spending Account (LPFSA). This account is only available if you enroll in the NVIDIA HSA, NVIDIA HSA Plus, Kaiser HSA or Blue HSA Gold medical plans, and is limited to eligible dental and vision expenses. You can contribute $120 to $2,600 in 2017.
  3. Day Care Flexible Spending Account (Day Care FSA). Use this account if you and your spouse work or attend school full-time to get reimbursed for care of eligible children under age 13 and adult day care expenses for your eligible family members. You can contribute $120 to $5,000 each year.

You’ll receive a Spending Account Card for the Health Care FSA or LPFSA. It’s a convenient way to pay for eligible expenses. Use it at any service provider that accepts credit cards.

Important! FSAs follow use-it-or-lose-it rules, per IRS guidelines. That means you must use the money in your account before the stated annual deadline or you will forfeit it.

Health Savings Account

If you enroll in the NVIDIA HSA, NVIDIA HSA Plus, Kaiser HSA or Blue HSA Gold medical plans, you get the added benefits of a Health Savings Account (HSA)—a savings account that lets you pay for eligible healthcare expenses using tax-free money. Use the money now to pay for care or reimburse yourself for expenses for you and eligible family members. Or pay your medical bills out of pocket and save the funds for later—even in retirement.

NV HEALTH Programs

NV HEALTH programs work to increase your health awareness and help you stay healthy by balancing the demands of work and life.

Genetic Testing

Know your risk. We’re partnering with the experts at Color Genomics to offer no cost testing and counseling to help you understand your risk for common heredity cancers (melanoma, breast, ovarian, pancreatic, prostate, colon, uterine, and stomach).

All you have to do is provide a simple saliva sample.

Your results are reviewed by a certified medical professional and emailed to you. Color Genomics makes it easy for you to share your results with your primary care doctor so that you can create a screening and prevention plan.

All testing is done through Color Genomics, no personal health information is shared with NVIDIA. As the field of cancer and genetics evolves, Color will keep you updated if any information related to your results changes.

GuidanceResources

When you need help with issues that affect your quality of life at home or at work, confidential assistance is just a phone call away, any time of day—at no cost. Services can be used by you and anyone else in your household, and include:

  • Counseling on relationship issues, depression, stress, grief, alcohol and drug dependency, family problems, and more
  • Pregnancy, child care, and senior care resources and referrals
  • Eldercare resources to help you plan for the care of an aging parent, family member, or spouse, relieving the burden of time-consuming research and providing qualified, realistic options
  • Adoption support and information
  • Legal and financial consultation
  • Career management advice

Receive up to eight counseling sessions with a GuidanceResource counselor per incident each year.

Health Risk Assessment

The Health Risk Assessment provides a snapshot of the state of your health and well-being. You’ll also receive an action plan to help improve your health risks. Before you complete it, you should know your current blood pressure, total cholesterol, and HDL cholesterol levels.
